Registration Summer Packets Available Monday July 17

Dear Parent/Guardians,
Our summer packet of information will be available to be picked up from the office starting Monday July 17th - Friday July 21st 7:30AM-3:30PM
Please come to the office to pick up the summer packet and complete the required info listed below. All summer packet information needs to be completed no later than Friday August 4th.
The summer packet of information contains information for you to keep at home and information that needs to be returned to the office.
Documents that Need to be Completed
New & Returning Students
  1. Student/Parent Handbook Agreement
  2. Proof of Residency (SDG&E, Cable, Home Phone, Water, or Lease Agreement) (All students)
  3. Immunization Records (New Students and 7th Graders Only)
  4. SDUSD Lunch Application
  5. 6 to 6 Application (If Applicable)
New Students Only
  1. Original Birth Certificate or Passport
  2. Health & Dental Examination Forms (TK, K, or new to California students only)
