Immunization (Shot) Requirements
State law requires that all students under age 18 years, pre-kindergarten- through grade 12, be immunized against- certain diseases unless they are exempt for medical reasons or because of person- al beliefs. At the time of registration, SDGVA is required to have proof that your child has received all currently due immunizations.
Students entering kindergarten should have:
o 5 DTaP 4 doses meet requirement if at least one was given on or after the 4th birthday
o 4 Polio 3 doses meet requirement if at least one was given on or after the 4th birthday
o 3 Hep B
o 2 MMR (1st dose on or after 1st birthday)
o 1 Varicella
Emergency Card Information
It is mandatory for the safety of your child that this card is signed and dated. The card is maintained in a file and used in the event of an emergency. SDGVA will not release students to anyone during an emergency situation if the name doesn’t appear on the card. Any change in address, telephone number, parent’s work, physician, and name of persons authorized to pick up your ill or injured child should be reported to the office immediately. Be sure to list at least three people on the school’s emergency cards that would be willing to assume the responsibility of guardianship in your absence. Be sure that these individuals acknowledge they will be responsible for your children until you get home.
